ONLINE REGISTRATION
Under registration there are 4 main forms to complete. Follow the instructions to make an account for you and your student.
Obtaining Physicals “form 3”
A current (within 1 year) CHSAA physical form (not a wellness check) must be uploaded to register. It is best to obtain these forms early before practice begins.
If an appointment is an issue with your family Dr, Eagle Urgent care has been outstanding. it is best to book an appointment online. Their fee is $30 (2025 prices).
Other options - Some Walgreens or Chiropractors
Please be sure to get a “CHSAA physical form”, not a wellness check.
Forms are in the front office or click “physical form” under announcements in the athletics page from WCMS website.
Payments* $75 fee for WCMS students, $100 fee for non WCMS students.
*the Family max for WCMS students is $225
